Social Worker - Caregiver Support Coordinator

Administratively responsible for the clinical programming and daily operations of the PCAFC. Manages and coordinates the application process for the PCAFC in Caregiver Support Program data system and electronic health records as appropriate. Conducts assessments of Family Caregiver applicants and participants. Provides appropriate clinical supports and resources as indicated. Ensures that caregiver training is made available to caregivers, and coordinates with the Caregiver Support Staff to ensure completion of clinical assessments, required home visits and monitoring within timeframe guidelines. Responds to all referrals and inquiries to include Caregiver Support Line, Health Eligibility Center, Health Resource Center, and VACO Caregiver Support Program. Serves as an advocate for ensuring availability of services and benefits for caregivers and Veteran s/he serves. Advises and collaborates with interdisciplinary teams throughout the VA medical facility, including leadership, on caregiver issues Educates Primary Care and Mental Health Services staff on the CSP and encourages all Patient Aligned Care Teams (PACTs) and Behavioral Health Interdisciplinary Program (BHIP) teams to engage the caregiver as part of the Veteran's health care teams Creates educational tools, develops programs and implements training focused on specific caregiver needs and issues. Provides education and in-services trainings to VA medical facility employees, Veterans, families and caregivers related to caregiver needs and the VA Caregiver Support Program. Conducts outreach, to include special events and activities, at least annually during the month of November in recognition of National Family Caregiver's month. Serves as resource to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs and respite services. Serves as a liaison between the VA and community agencies. Participates in caregiver support activities in the local community and state to promote and recognize the role of the caregiver. Develops a data monitoring system to measure the efficacy of the program. Collects, analyzes and reports program data to local management, VISN Caregiver Support Coordinator Lead and the National Caregiver Support Program, Care Management and Social Work Service, Office of Patient Care Services. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Experience:
To be creditable, the experience must have required the use of knowledge, skills, abilities, and other characteristics associated with current professional social work practice. The experience or education must be post-MSW degree. Experience and education satisfying this requirement must be active professional practice, which is paid/non-paid employment as a professional social worker, as defined by the appropriate state licensing board. Experience is only creditable if it is obtained following graduation with a master's degree in social work and if it includes work as a professional social worker directly related to the position to be filled. Qualifying experience must also be at a level comparable to social work experience at the next lower level. Part-time experience as a professional social worker is creditable according to its relationship to the full-time workweek. Fellowships or Post-Graduate Training. Fellowship and post-graduate training programs are typically in a specialized area of clinical practice, i.e., group or family practice. Training as a fellow or post-graduate may be substituted for creditable experience on a year-for-year basis. Experience, Education and Licensure (GS-12 Senior Social Worker):
The candidate must have at least one (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities:

(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). The actual grade at which an applicant may be selected for this vacancy is in the range of GS-12 to GS-12. Physical Requirements:
Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others. Position requires walking throughout the clinic and the Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. Position requires occasional travel into the community to meet with community agencies and partners who assist and support to the Caregiver Support program, its Veterans and caregivers. This position also requires close visual acuity to perform an activity such as:
preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
